Oakleaf offer a range of specialist services designed to meet the needs of adult males with complex neurological conditions and brain injury. All services are supported by our highly skilled and experienced multi-disciplinary team providing a flexible and seamless rehabilitation pathway.
Referrals can be made to the service by using the online referral form via our website or contacting the main site directly. Assessments are offered free of charge and referrers will receive a comprehensive report with detailed findings, recommendations and initial goal setting expectations. Our services are able to accommodate those on Section 17 Leave of the Mental Health Act and those on Community and Guardianship Orders.
The large multi-disciplinary team consists of a wide range of healthcare specialists and effective interdisciplinary working is paramount to the development and success of the individual programmes of care. We benefit from on-site· 24 hour nursing (RMN, RNLD and RGNJ) which enables the service to accommodate more complex nursing needs including Percutaneous Endoscopic Gastrostomy (PEGJ feed), tracheostomy, palliative care and end of life care. Other on-site specialisms include Neuropsychiatry, Neuropsychology, Occupational Therapy, Physiotherapy, Speech and Language Therapy, Rehabilitation Assistants, Family Liaison Team, Horticultural Therapy and access to a dedicated GP service for all residents.
